Hello,

Today I got a PIP Review form through the post, do I use your normal PIP guide for this? As I can see from your guide I do not have the questions on the Review form.

Also, It says the form needs to be back by the 12th February, but the form only arrived today which give me less than 4 weeks to complete and return. The form is dated the 12th Jan.

The four weeks to complete the form is from the date of the letter (including the time it takes to arrive!)
