We had some help from a Wefare Advice Centre to fill in a DLA to PIP Transfer Form A Face to Face Assesment is planned for next week. Is it wise to take all medications to the appointment or is it sufficent to just take a copy of the prescription. Also how do we get a copy of the report .
I am hoping to start reading the members guide tomorrow
My partner has had a DLA indefinate award for Mobility since 1997

No, a current repeat prescription should be sufficient.

You should be able to request a copy of the report a couple of days after the assessment by contacting the DWP office dealing with your claim, contact information should be on the letters that you have received about the claim.
