Pip review form 6 months prior to award end date

Hi wondering if anyone has any advice. My son has an ESA and PIP award, both ending this year, and has now received the PIP review form for return, If anything he is worse than when he started. The form says might not need a further assessment depending on form reply, we will fill it and return but there isn't a lot of additional enclosures we can include other than a letter as carers as he can't face talking to counseling etc, and the doctor only really prints of his repeat prescription every 2 months without going into much Wondering if any general advice, of course rather avoid the stress of going through it all again,we are almost wondering if we point out ways he is worse, that might even more likely to cause another assessment than if say he's exactly the same!

Hi have realised that he lost his old mobile ages ago and the one we have been texting him on since he keeps in his place. Not sure if should mention that number and the landline to dwp as they might say its an uninformed change. (he leaves the landline unplugged as the ringing sound annoys him)
