Hello, I have a face to face on 20th March. I have just looked at my online GP medical record and have noticed that PIP have sent a form to my GP for completion. I am a little concerned as my current GP does not know me as well as my GP who recently moved away. What questions will be on the form. Is there a copy on this site. I had so much evidence, some old as well as up to date that I sent with my application that I wasn't expecting them to contact my GP especially as I didn't think they contacted the GP under normal circumstances unless for appeal purposes. Should I see my GP to see what he has said or just leave it and see what happens. Please advise if you can.

Hi furry
The DWP only contact a small number of GPs for evidence about people's health in support of benefit claims.
If the DWP send out a form to the GP making a request for medical information - GPs are supposed to complete it and there is information available on what they should say.

Thank you Gary. It does concern me as I know without realising, GPs can easily lose someone an award by not knowing home and personal circumstances and therefore not completing the questions fully. I will have to wait and see.
