Confused about PIP Review Form was created by NathalieSheridan
My PIP award finishes at the end of August (awarded in 2017).
I received a form recently (PIP 1043) and thought it was the regular application. It was only when I photocopied it, I noticed it was 24 pages whereas I think the previous one was 36?
It was called: Award Review - How Your Disability Affects You and was very precise as to what sort of background medical evidence you can send in, e.g. no letter from G.P. and no information you've already submitted in your previous form.
Is this a different form to the regular PIP application form? Should it be completed in a different way?
I'd be grateful for any information please.

Hi NathalieS
You have received a PIP review form, it is different from the regular application, complete it in the same way as a regular application and don't forget to add any supporting documents you may have and any current prescription lists.
