misleading notes from the assessment

misleading notes from the assessment was created by Mitch61
justing writing notes for a MR and looking at the activity engagement with others face to face.
at the start of my hubby's assessment when the guy first walked into our lounge he greeting my hushand in the loud and fast talking manner and my husband asked him to lower his tone as he cant cope with the noise levels, the assessor immediately said 'do you want me to leave' my husband said know its just that your loud. He began to explaining the purpose of the visit and again my husband asked him to lower to voice and he again said do you want me to leave ( he seemed to take offence, which did not fill me with confidence, as a nurse he should have the skills and knowledge to adapt his why of communicating to deescalate a situation and continue with the therapeutic relationship increasing his rapport).
anyway my point is that he did not refer to this situation in the report other than to say under the section General appearance and informal observation that my husband was rude to him on two occasions for which he apologised for at the end.
noise is a difficult senory challange for him and he does get angry very quickly if under pressure and feeling stresssed.
he was awarded 2 point in 2017 assessment under this activity and 0 this time.
should i mention ant of the above. the assessors notes under several section are notable wrong, but i need to keep focused

Hi Michele
You want to concentrate on the areas where your husband didn't score points or not as many as he thought he should do so for the specific PIP criteria.
Are you talking about Q11? If your husband suffers from noise sensitivity and this is part of a medical condition, then you should emphasise the effect it has coupled with what happens when he is put in stressful situations and you should give examples.
With regard to the assessor and their attitude - I would mention it, but I would do so at the end of your comments where you are challenging against the specific criteria. You could say that you were disappointed that the assessor didn't seem to recognise the difficulties associated with noise sensitivity which your husband pointed out twice right at the beginning of the assessment etc etc.
If there are lots of mistakes in the report - you are also free to put in a separate complaint to the DWP. It won't have an impact on your MR as they are dealt with separately by different people.

The guide is based on the PIP2 which has two extra questions of the form, 1 and 2, so the activities are questions 3 through 14, it sounds like you have the AR1 Review form which just has the activity questions, 1-12, so just add two to the question number to find the correct entry in the guide or go by the question headings which should be the same.
