UC50 form extension due to pandemic/lockdown was created by Dom
I, like many others, will clearly need to request an extension as I have no way of getting my supporting evidence together under these circumstances. I have no access to a photocopier, and mum mum who will write a supporting letter is in self-isolation.
Who do I need to contact? I don't see a number on the form or on the letter from the Health Assessment Advisory Service.
If they grant an extension, what proof should I get of this?

The UC legislation does not specifically provide any extension to the four weeks allowed to return the UC50, however, unlike the ESA legislation it does not specifically restrict it to only four weeks, so it may be possible to request an extension but I have concerns as to whether the call centre staff actually have the authority to allow one.
My advice, given the delays that are already in the assessment process and the likelihood that these will extend at least in the short term, is to return the form on time so that you avoid any issues that sending it late might provoke and then send the evidence as soon as you can.
Make sure you take note of the return address so you can send the information to the same place and I would also send a copy with a covering letter to the DWP office dealing with your claim, their address should be on any letter that you have received about your UC.
