Your ESA will not end in November, the date you have been given is when your award will be reassessed. ESA is an indefinite award and can only be terminated following a Decision to do so. Assuming things will be back to normal by then, I would expect you to receive an ESA50, whilst there is a short form it is very rarely used, around the end of October.
For your PIP, if your current award was for more than two years then it will be the AR1 Review form, if it was less then it will be a new PIP2, if it actually was for two years then it could be either.
However, I think you will be caught by the current three month suspension of new reassessments and your current award will be extended, probably by a year, the form was being sent six months before the end date.
