How do you explain a complex physical neurological disease that causes psychological issues such as overwhelming psychological distress to a DM?
Engaging with people, socially aggravates the condition.
I was previously awarded no points on the engaging face to face category despite providing proof of a visit to neuro psychology and a supporting letter from a specialist telling me there was nothing they could do?
In most people with the condition there is a 60% recovery rate if caught in time. However, I have two other conditions preventing the disease which I am describing from going away-for which there is no known cause.

You need to deal with three issues.
1. First, you need to provide evidence to support your problems. You say that you have done this but you should direct the DMs attention to it nonetheless.
2. you need to explain how these conditions limit your ability to complete the PIP activities, don't assume that just because you have said "abc" that they will make the leap to "xyz", you need to lead them to it, explain in detail and don't be afraid to repeat yourself.
3. Lastly, you must address the Descriptors, you can show a significant level of disability but if you have not explained them in terms that relate to the relevant Descriptors then you won't score a thing.
If you have questions about the Descriptors then the first place to look is the PIP Claim guide but if you do not find an answer there then ask in the forum.
