PIP - Daftest question ever? was created by Ghirl#1

I'm new to the entire benefits world. Have worked for 40+ years and now need some help. Been going round in circles trying to find out how to download a copy of what the PIP application form to see what the form looks like. Can anyone provide a link? I can find the ESA application and download that no problem, but PIP? Pfffff!
