I have been sent form RARL2A. I had initially requested info about my son's recent telephone assessment and they have sent me this form. They have detailed the info I initially requested but are asking if I require any other information and to detail this in the given space. Please can you advise on the kind of information I am allowed access to regarding my son's claim?
Ideally I would like all information they have including medical information from GP etc, but unsure f this can e shared with me.

Do you actually mean a Freedom of Information request, this is used to request bulk information from the DWP, for example; the number of people claiming PIP.

To request personal information you need to make a Subject Access Request.

Is your son under 16 or are you their appointee? If not then legally you cannot make the request only your son can.

If this is about a Decision then ask for a copy of the documents used in the making of that Decision, otherwise, you can ask for specific documents or documents on a particular date.

Thank you Gordon,
I am my son's appointee. The information I initially requested was notes on a telephone assessment for esa where no decision has been made but they have informed a face to face will now be required. I have complained about this due to unnecessary stress. I wanted to know if I could ask for all personal information held in his file from beginning og the claim in 2018? Do I need to specify the documents I wish access to, as I'm not sure what they have or can I just ask for all personal information including all reports?

Unless the notes were submitted as a report to the DWP, which I doubt, then you won't be able to get access to them, the DWP is the data owner not the Assessment Provider so they can only supply information that they have and as the AP is not a Data Owner you cannot request information from them.

The documents that will typically exist are;

- the claim form and the evidence attached
- the assessment report
- documents sourced by the assessor
- forms completed by the assessor in regard to telephone conversations that they had with medical contacts.
