Is the CRMR1 form required for MR? was created by SirSupaDupa
I've called PIP to request an MR and the assessment report for both, I told them i want an MR on my daily & mobility and told them i'd be sending in further evidence and a letter stating my reasons for this.

I've now a got a letter supporting me and stating my reasons and why i disagree with the help of CAB writing it up for me + my new evidence from physiotherapy/GP letter.

So my question is do I still need to fill out the CMMR1 or not? I'd rather not be honest if i don't need to.

Hi Sam

Whenever I apply for a MR I use a CRMR1 form but you do not have to use it.
How to ask for mandatory reconsideration

Contact the benefits office that gave you the decision. You can contact them: by phone
by letter
by filling in and returning a form
The contact details are on your decision letter.
