Hi guys,
Following my telephone assessment in July I received my letter advising that I had not been successful in claiming PIP. I am now going to start the process of a MR. I have read your guides so apologies for the questions as I know they are probably already covered in them. Do I just use the letter template to request it or is there a specific form?
I have the assessors report, and he has seemed to pick bits of information from our call to support his decision but missed out the majority of what I said. Do I challenge this? Can I send in extra information i have received since I made my claim? As I was only given a days notice and had to do the assessment alone, I have a friend that cares for me alot who wanted to add a letter, is this allowed?
I am claiming solely on mental health grounds which I know is difficult, especially on the mobility section.
Any help you could offer would be greatly appreciated.

You can either use the government form for your Mandatory Reconsideration which you can find in the link below
CRMR1 form
You can just write a letter with your name and NI on it - clearly marking it Mandatory Reconsideration or phone and request a MR. I usually use the CRMR1 form.
Have a look at our PIP MR & Appeal guide for details of the process, the PIP area also has template letters that you can use to make the request with.

Your primary task is to show that you meet the criteria, there are many reasons you may have failed, you need to address each of these but don't get bogged down in criticising the assessment report unless you can clearly show that it is incorrect, it is a lot easier to argue the facts of the situation. Go through the assessment report line by line, highlighting anything you disagree with and why you disagree with the report.
Yes you can send in extra information if it supports your claim and also get your friend to write a supporting letter on what help they give you and why. They will need to sign, date the letter and add their address.
When you have a better idea of the issues with your claim, come back to the forum and we will do our best to help.
