Can I ask is there any guidance for filling in this review form on this site, as I cant seem to find it. This form looks slightly different to my last one which was basically tick boxes.
Also when asking for a 14 day extension to return forms do I make sure I get a reference or name as back up

page 102 is the start of guidance on the reviews.
If you can ask for a reference or name as back up when you ask for an extension it is has to be helpful if it is later disputed, although if you forget I would not worry excessively. It you want even harder evidence you could always record the call.
