Can you please tell me is there a Universal Credit form I can look at before I apply for it, if so where can I see it? and what is the difference in forms between the ESA 50 and the UC50?

Hello sam,
There are two possible forms you could be referring to.
The first, which is to make an initial application for UC is filled in online and is very straight forward. It is just a series of questions such as name, NI number etc. There are not many questions to each page and the answers are short. Once you have started filling it in you can take a break and go back to it.
The form which I suspect you are thinking of is the UC50. This is the Capability for Work Questionnaire. There is no difference between the ESA50 and UC50. ESA50 is for people who are receiving ESA and UC50 is for people who are receiving UC.
It is possible to download this form as a pdf and either print it off and fill it in by hand, or fill it in on your computer and then print it off. This is a link to it.

What I often do when I am filling in one of these forms is type in the answers into a word document which I can then think about before transferring it to the form. That also has the advantage of having a spell checker and grammar checker! When I am happy with what I have put I copy and paste it into the form.
You cannot send the form in until you have been asked to do so, but, assuming your condition is stable, there is no reason not to start filling it in now.
Note: I have sometimes had problems saving the form, depending on which computer I am using. So if you do decide to go down the route of filling it in on the computer, I suggest that you save it regularly and check that it has saved by closing and opening it again.

I was formely known as (GoingOffMyHeadWithThisGovernment) Won PIP November 2017 ongoing award HR both. Now kept and got my ESA Support group but took a while.

Also Sam do as Catherine has already said, to note any extra added paperwork, and any medical evidence your sending in, make sure it has your National Insurance, and name on any added paperwork.
