Hi Neil,
To answer your initial query, have DWP taken things into account if not on the list? - my advice would be to assume not, unless there is a proper reference to this within the report. On which basis resend the missing documents and explain these when you do MR!

As I believe there to be no reference to this I have assumed not and have today sent in a mandatory request which I have included the information that had been sent in and not listed as well as further medical evidence and some new evidence.

Hi Neil, Denby here. So sorry for the late reply, my husband has been in Hosp for an op so I have been less attentive than I would like. [he's home now]. What I meant was, "As Soon As Possible, before the Decision Maker bases their decision-on-what-award-to-give-you on the PA4 you described, that has things missing &/or incorrect."
I only learnt myself from this forum that a] you could ask for the report, rather than just wait in agony for a made decision, and
b] that having asked for the form you could then write in to contest it as necessary.
A point to note is not to miss the one-month MR deadline if it is necessary to put in for one, as you have now done. Hence the ASAP bit.
I do make an effort to spread this knowledge wider, as people are coming new to the forum every day who won't have read all the old posts. It has been invaluable for our family as by writing in to set the record straight over the errors, twistings and wrong assumptions made in our daughter's report, she got Enhanced x 2.
