In general, do I have a right to ask my GP to provide a copy of his replies on the PIP medical information form?
Is a GP obliged to store his completed form in my medical notes?

Hi Finni
You are entitled to see any of your medical records. I would expect a doctor to keep a record of anything that is sent out regarding a patient.

My question is really outside of the Data Protection Act.
For instance, the time before my GP sent back the filled form to the DWP and didn't save a copy on my medical notes at all. My data rights don't arise if he does that because he doesn't hold the data, which is why I wonder if I'm entitiled to a copy before he sends any form back.

This is something you'd need to talk to your doctor about. Mine don't send off any information without keeping a record.

...but whether our daughter's GP kept a record or not, they were not wiling to tell me about it, even though I have daughter's authority to discuss/disclose. I found out roughly what they had put from the PA4 later - which was "too many questions to answer so not answering any" ! I did not mind or bother pushing it as his letter had got her a paper-based assessment, and after I wrote and took apart all the lies etc in the PA4 she got her Enhanced on both again.
