Hi to all,
I may be wrong, but somewhere in the past, i'm sure in the guides there was a template, which could be used to take to your GP. The template was so that the GP would write ......
My patient Mr fibro cannot attend a PIP assessment, due to physical pain. And also due to his severe depression and anxiety, it would cause him overwhelm him psychologically.
Therefore a paper back review would better suit his needs.

There are the GP notes - which asks them to comment on the different PIP criteria. We definitely recommend only taking the pages that are relevant

There isn't a template letter for the doctors. You will have to request this yourself. Without the doctor saying that a face to face assessment would cause you "overwhelming psychological distress" or confirming that you don't have the mental capacity for a face to face assessment, the DWP will not necessarily accept the request (and they don't legally have to).

Hi Bis,
thanks for your reply. I am putting a letter together for my GP, which is a VERY, very short version of the answers to my upcoming PIP review questions .
And basically giving / showing my GP a more detailed insight on how my disabilities affect my daily living and my mobility. And my GP will form his own opinion and write me in his words a letter of support for my PIP review.
Finally I will ask him if he could also write that it would be reasonable for me only to have a Paper review given my multiple health conditions and disabilities.
My GP may or may not agree. But asking can only help right?

When requesting a paper review your doctor needs to add the phrase " that you suffer from overwhelming psychological distress"
