Hi Teddington
We say to members to follow up any telephone call request in writing as sometimes telephone calls are not recorded, so this advice is to cover yourself. Yes it can be short.
All you have to say is give your name address and NI number and say further to my telephone call dated ........ I spoke to ....... who confirmed that a copy of my assessors report (PA4) will be sent to me.
When you get the report you need to detail any errors in the report and try and tie them in with any supporting documents you sent in.

Hi,
Finally my assessment report arrived .
As I expected, assessor missed out important parts of my medical history and made wrong assumptions.
He explained my health problems like they are nothing !
In my MR letter, can I pointed my previous reviews with their points ?
Can I say like " I scored 8 points for this activity previously. My difficulties have not changed but I scored 2 points this time " etc.
How can I say this very effectively?
Rest of the assessment report missing what I told him or he contradicted what I actually said.
Any other advice you can give me ?
I am so thankful to your help. I don't know what I would do without your help .
Unfortunately, I couldn't get new evidence ( like new doctor letters etc) . Is that so bad ?

He is a paramedic.
I have life long auto immune disorders, chronic pains etc.
How can he know about my complexity of my health conditions??

I am pleased to hear that you have your assessment report at last.
Yes, you certainly can, and should, point out what you were awarded in the past. Your suggestion sounds spot on, I would say something like ‘following my assessment on xx/yy/zz I scored 8 points for this activity. My condition(s) have not changed since that date and my difficulties remain the same.’
You can put in something like ‘At the assessment I told the assessor that I was unable to …. , but this has not been recorded. I would be grateful if you would take this into consideration when reconsidering the decision.’ Or ‘The HCP incorrectly recorded that I told him …. . The reality is that …… ‘.
As you go through the activities, don’t be afraid of repeating yourself!
And Teddington, please do not apologise for how you feel. We all know what it can be like and are here to support you as much as we can.

Teddington I just want you to know you are not in the very least being rude. The only rudeness around you and your claim so far is the assessor who plainly wants to please his/her bosses at the expense of the truth and of increasing your stress which is known to be bad for auto-immune conditions and many others!
Grab any help you can from friends and family to keep you in meals, cups of tea etc while you get this MR written well as you and Mods outline and sent, then rest up as best you can, you deserve some recovery time. [Be sure to keep a copy and send signed for] And even if there isn't anyone to support you on the spot we are all willing you on here as we all know something of what this c__p is like, excuse the strong language, they deserve it.
