supporting someone to fill out the PIP form

Hi,

could you please advise me if it is acceptable when completing the form on behalf of someone to write it in this way:

Joe requires assistance with this acivity
This is becasue Joe.....
If Joe was not to have this assistance....

This is the way we are writing it and we are typing it up so it relates to the question and page number of the question instead of hand writing it.

Just wondering if the above is acceptable.

The reason for writing it in this way is due to the claimant not being able to write it themselves and also due to the fact of the claimant having such low mood, depression they are not engaging fully in it.

Is it best to send a supporting letter alongisde this to explain the reason that we have written it like this?

Hi Anne,
It is perfectly OK to write the form 'Joe can not do X because...' The important thing is to make it clear in the notes "This form has been written by Anne on behalf of Joe,' and explain why.
Write a separate statement explaining how you know Joe and the care/assistance he needs as a result of his disabilities. Remember that only activities within the descriptors are relevant, so things like housework or gardening etc are not relevant.
Under each descriptor, give examples of Joe's problems, and what goes wrong etc eg last time he cooked a meal he burnt his hand, or 2 weeks ago he fell over and needed stitches in his leg etc etc. Use extra pages if necessary but attach these well, and indicate you have added a page. Include name and NI number on all new pages. If you are typing answers, just write in the box 'see separate page' and make it clear where the answer is.
The members guides give plenty of info about firm filling. Please have a look at them, if not done do already.
