I received a PIP2 form on 1/6/21 and am using your online PIP guide (June 21 v.50) to help me to fill it in. However, the questions on the printed form do not seem to directly align with the online guide:
eg The online guide says: Q1 Please tell us who are the professional(s) best placed to advise us on your circumstances. For example, a GP, hospital doctor, specialist nurse, community psychiatric nurse, occupational therapist, physiotherapist, social worker, counsellor, or support worker?
Whilst the form I have received says: Q1A – tell us in the space below: • What health condition or disability you have? • The approximate start date
So has the form changed and you've not yet been able to update your guide or have I been sent the wrong form? It says PIP2 in the footer.
There is a new form out and our guide is going to be updated to reflect the changes. The new form was only being used in some parts of the country at some point, but it now seems it is being sent to everyone. The guide is still relevant to how to answer the specific PIP questions.
