Hi there
I threw out all past photocopies of forms when ill.
So, the original pip form, and the follow up AR1 form (Better/Worse/No Change)
1. I am expecting a review form now, will it be an Ar1 form again, or could they make me fill out another original pip form?
2. And How do I fill out the AR1 form if I don't have any forms to refer back to because I have thrown them out?

Hi

You can put in a request to access the information from the DWP. It may take some time, but you could ask for a copy of your previous form and the report. Details can be found here

If you can't do this you can only write what you remember.

You should receive and AR1 form
