Hi there I threw out all past photocopies of forms when ill. So, the original pip form, and the follow up AR1 form (Better/Worse/No Change) 1. I am expecting a review form now, will it be an Ar1 form again, or could they make me fill out another original pip form? 2. And How do I fill out the AR1 form if I don't have any forms to refer back to because I have thrown them out? Thankyou
You can put in a request to access the information from the DWP. It may take some time, but you could ask for a copy of your previous form and the report. Details can be found here
