Pip2 questionnaire was created by KazM
Is the Pip2 questionnaire a new form? It's just in a reply to my complaint it says the information was consistent with information supplied in my Pip2 but I only completed my review and my assessment 3 years ago. I don't recall seeing a pip2.

Hi Karen
If you meet the basic conditions you will be sent a PIP form called ‘How your disability affects you’ by post, it's also known as the 'PIP2 questionnaire'.
A PIP renewal form is called an AR1.
The PIP2 form would have been the first form you completed when applying for PIP. For some claimants who win at a Tribunal they will have to complete a PIP2 form when they re-new their claim.
