PIP asking for a doctors support letter

Hi can anyone please offer me some advise please, on what to ask my sons doctor in terms of supporting medical evidence report to send with a new pip form.

I have been advised by a local service, that a doctors support letter is important and can request evidence of my sons mental health condition and drug addiction, which would include my sons medication history and referrals to Mental Health therapies, appointments , Drug specialist support and Residential drug Rehabilitation Clinics etc, but also to give him a copy of the pip descriptors or templates and get him to make his comments and complete the ones that are relevant to my son.

However , the difficulty is the doctor does not know everything about how each descriptors effect him on a daily basis.
So, I was willing to do him a letter highlighting each criteria and my sons statement on how these these do effect my son or giving a copy of my sons completed PIP form to help him complete the descriptors and keep on his file.

I was advise that I shouldn't do this, because they may be a possibility that if my sons case goes to a tribunal, they could ask for to see what letters or information I have given the doctor and they look out for whether the report was based on our views or the opinions of the doctors.

So the dilemma is dont give the doctor any information other than requesting medical history and he wont be able to fully answer the descriptors if I just give him the template. This is so confusing and stressful, as I have spent days worrying about this and yet haven't given anything to the doctor and time is running out.

If you look in the guide tab above, under PIP you will come across

PIP GP Notes
A copy of the PIP descriptors you can take to your GP to ask them to select which ones they think apply to your son, add any comments and sign and date as evidence for his claim or appeal. Just take the pages you need.

You can always request a copy of your son's summary report from the GP, this outlines any medical issue he has and any appointments he has made?

Hi Gary, sorry, I have not replied I've been away for two weeks. Thank you for your advise, I have downloaded the example letter showing the PIP descriptors. I have included a personal additional information letter which covers all of the descriptors and how these effect our son on a day to day basis, hope this is alright to do this for the doctor and has no repercussions. thanks again.

Hello Carer. I know it’s a bit late to the discussion, but were you aware that your son has the right to access his medical records held by his GP/ Medical Practice?

Going forward, they would be a very useful source of corroborating information and supporting documentation that claimants are/have been in contact with their GP/nurse/health practitioner about their ongoing health issues etc. Also, it is possible to provide a history of medications that may have been prescribed.

I would suggest that due to time constraints etc, GP’s would not have the time to read your sons completed PIP application form. Good luck.
