How to Add on PIP review Form Previous Award

I've seen that adding your previous award and what the assessor had written in her form about why she scored points on each category,

How do I add this onto my review form
If I add a sheet of extra paper I'm stuck on how to put it without annoying them

I'm worried they might take offence as in "we know what you were awarded and what the assessor said its on our screen!!"

Hi Positivity 2016,

You can write whatever you need to write on the extra info page attaching anything more on sheets of paper.

Yes DWP know what your previous award consisted of, but there is no harm in writing anything relevant linked to your current application.
