PIP review form question was created by must_not_tell_lies
hi, im currently having my PIP reviewed.
Does this sound like an appropiate answer template to use. I would try to write more but I am also under review for my ESA at the same time so currently not really capable to do much more.

There has been no change from the reported problems with ________ that I received __ points for in my previous award, dated, ________, where I received ____points. I am still in need_________________________________, as I can not do this safely, reliably, repeatedly and to an appropriate standard due to my disabling conditions of ___________. This is confirmed by __________ supporting evidence and evidence supplied in relation to my ESA claim that is being reviewed co-currently. I also enclose ________________as additional evidence of my continued problems in this area .I expect this to continue for the next 5 years due to the long term nature of my problems in this area.

thanks. obviously going to add appropiate infromation in the blank bits.

Hi must-not-tell-lies

That sounds fine, but I wouldn't put you expect this to continue for the next 5 years -because you are inviting them not to give you a longer award. They may not give you a longer award, but if you suggest five years - the review form will come anytime after four years.
