DLA Renweal Form - Download available? was created by paulcornwall
We've just started to look at our daughters DLA renewal. The only download I can find is DLA1 (ALT) and its different to the physical form. Can someone direct me to the correct form please?

Hi, and btw if you are using your real name you are advised to change it, are you looking for a copy of the physical form to help you draft your responses?

Strangely, its a 24 page form so much shorter than the DLA1. I've scanned the pages so I can type out the answers.
