Migration from ESA to UC

I have recieved the forms to migrate and have no idea of what to do.
My husband has just been diagnosed stage 4 lung cancer and I am at my wits end.
I have rung both Age Uk and CAB and both give different answers.
I am 61 & my husband is 71. He is on my ESA claim, but recieves state pension which is deducted from my ESA.
We both get SDP and Carers premium.
I have looked through a dummy form for migrating.
The question " do you claim carers allowance" how do I answer, as we cant both get the premium and theres no option for premium on the form.
Also where do you tell them on the form we both get SDP? I've either missed it or its not on the dummy form.
To say I'm at my ends with worry, my husband as only just starting treatment for lung cancer and I'm terrified for him and trying to sort these forms out, but my minds blank.
Deadlines 21st June.
Any help appreciated.

I would not contact your local Citizens Advice office. You should phone the Citizens Advice Help to Claim phone number which is on page 2 of your Managed Migration letter.

Thats who I've spoken to, the CAB Help to claim line. Who didnt know the answer to my questions.

I just read that link & cant see anything about the carers question & the SDP question.
