Appointee Query: Claimant Refusing to Sign UC50

I am the Appointee for my partner who has very severe dyslexia and most likely other neuro developmental problems currently undiagnosed. I have spent some time completing their UC50 giving a true reflection of their difficulties. I have included references to their emotional dysregulation and examples of incidents. However, having read the completed form to my partner they are refusing to sign it. They are in denial about the extent of their difficulties and fear legal repercussions from its contents. They have said that its contents are not true, even though sadly they are. I understand that as their Appointee I can submit the UC50 without their signature, although I would rather they were cooperative. This is typical of their self sabotaging behaviours. We have had a serious incident before where medical assistance was refused in a potentially life threatening situation. I am tempted to add a note to say their reason for refusal for signing and their self sabotaging behaviours. In some way it may actually add weight to the application.

Any advice would be welcome.

Thanks for that. I hadn't realised that the appointee signs the forms, as it says when you fill it in for someone else where possible they should sign. The instructions are not clear on this. That's all good then. Saves a drama. With this in mind, I had filled the form in in the first person. I'm assuming then it should be then in the third person. And also there is no need to complete the section 'people filling in this questionnaire for someone else' because I am the appointee.
