On the online claim, there isn't space to explain anything next to where you put the health conditions. You literally just add the conditions one at a time and it's just the name of the condition. You can see all the questions in the order they appear on the form on page 18 of the ESA to UC migration guide.
If you refer to the bottom of page 18 and top of page 19 of the guide, we suggest that you add a note to your journal highlighting that you are in the ESA support group. This is how the note is worded in the guide:
Please be aware that I am migrating from ESA and that I am therefore not required to provide fit notes or undergo a WCA. Instead, the law requires that my WCA status for ESA must be automatically transferred to my UC claim. For further details, please see Regulation 19 of The Universal Credit (Transitional Provisions) Regulations 2014; Regulations 39 and 40 of the Universal Credit Regulations 2013; the Advice Guide for Decision Makers, Chapter M6: Effects of transition to UC.
It is not common for the process of managed migration to trigger any sort of reassessment of your health issues especially since November 2024 when the UC claim form was amended to allow claimants to indicate that they are currently in receipt of ESA. If you do find yourself wrongly being asked to provide a fit note or undergo an assessment before the LCWRA element can be added, you can challenge this via a mandatory reconsideration and there is further info about that in the guide.
