Yes/no questions on newpip form

I'm trying to help my daughter with her new pip review form. Many of the questions only offer yes or no boxes, not sometimes. She has a fluctuating mental health condition. So the correct answer is sometimes. Do I tick the yes box and then explain in the comments section?
