Is the claimant allowed to request a copy of the actual report the assessor writes which determines whether they are granted PIP ot not?

Hi Mark33

If you had a telephone, video or face-to face assessment - then you can call the DWP and ask for a copy of the assessment report. It is called a PA4. If a paper-based assessment was carried out they seem to have stopped to an actualy report.
