  			 				Thank you for providing the Guides and information to Members.
 
My question is a simple one:
Will my claim be refused or returned if I write a single "Please see attached sheets" on the ESA50 Form, sign and Date the Form, and then type and print responses on separate sheets to accompany evidence?
 
I cannot write legibly with a Pen due to Functional Hand Tremors.
I can mark a box with an X, but some I would prefer not to.
For example, I would prefer to leave the boxes about walking distance unchecked because I can walk nowhere near 50 metres.

  			 				I was going to leave all of the big white text boxes empty and write the information on attached sheets.
I was also going to leave some tick boxes empty as suggested in your guides.
For example: Walking Up to 50 Metres.

You can leave all the boxes empty - but I would put in the white boxes "see attached sheet No. x and make sure that you number all sheets. You want to make it as easy and as clear as possible for the person looking at the information.

  			 				Adding a bit to BIS's response, I would also suggest numbering the paragraphs and pointing the decision maker directly to the specific paragraph on any give page.
 
A while ago now, I settled down and wrote (by wrote, mean typed out on my laptop) out all my symptoms and their triggers (as far as I could establish them), which means when anything needs amending because of another review, I just have to edit those pages, rather than write everything again. When I have to answer questions on the form, I put the barest minimum on the form, then "see page(s) x paragraph(s) y, z, *, * " as necessary.
 
I also make a point of noting that I've taken the time and trouble to correct all the errors - if I leave my typing uncorrected for co-ordination and misfiring brain issue, my typing is incomprehensible a lot of the time. (When I fire up spell check, it wants to know what language I'm using before it can start picking up the errors.)
