Attd 1st commitments appt with clmt.

he has 3 chronic conditions all uncontrolled my meds or therapy . just finished college and looking for work.
Given the impact of his conditions i was unclear about how to trigger wCA. the case worker didnt know how this works without a fit note .

we were sign posted to
option 1 - get a fit note
option 2 upload medical evidence and someone will decide if this will be sufficient for wca trigger

feels like a blind alley tbh which is frustrating as its not knew to dwp jobctr bods

Hi Sum

To start the process they need to provide 'evidence' of their health condition. They can do this by reporting the change in their health condition on their UC account. This counts as evidence - referred to as 'self certification' and lasts seven days. The claimant will then need to provide more formal evidence - usually by providing a 'fit note'.

They will then need to provide a 'Fit notes', also called a Statement of Fitness for Work (SoFfW), Med 3, sick note, med cert and doctor's note. These can be issued by a doctor, nurse, physiotherapist, occupational therapist or pharmacist.

The DWP should be willing to accept other medical evidence, such as a doctor's letter, a SRI (terminally ill declaration), a BASRiS form (Benefits Assessment for Special Rules in Scotland), a Med 10 (hospital inpatient form), a psychiatric hospital admission form, hospital discharge letter and private medical certificates.
