Hi, I’m hoping for some advice if possible, please. I’ve just received a PIP review form. I haven’t had to complete one for some years and am a bit out of date and anxious.
I hope someone may be able to advise me on the following:
- they have sent a paper form and no link to download anything. Is it possible to download a form and complete it on the computer? Does anyone have the link, if there is such a thing?
- I received the form yesterday and it says I have to have it back with them by 15/12. This seems an incredibly tight timescale. Is this usual?
- on the form they make suggestions about what supporting information to include. One is something I have never heard of, ‘a summary of your patient record’. Does anyone know what this is, how to get it, how useful it’s likely to be?
- I used to see 2 Consultants regularly on the NHS for many years. They were very supportive at providing information for me for the DWP. Since Covid this has completely stopped. Effectively, I think I’ve been ‘parked’. Any advice on what to do in the absence of such information? Should I send ‘old’ supporting letters?

You are not able to download a form, some areas can apply online, we have heard from members who have completed the online form, have advised members to save the form as you go on because the system does not allow you to save the form when you get to the end.
If you kept a copy of your previous PIP review form, then use it as a template and make any relevant changes such as medication, new conditions (need to have had a condition for 3 months on going for over 9 months).
If you require extra time to complete the form, contact PIP before the deadline and ask for extra time, you usually get 2 weeks, we have heard from members who have asked numerous times for extensions but you cannot guarantee that multiple requests will be granted.
If the medical reports/supporting documents are still relevant then use them.
