Is anyone else struggling to know how to answer the questions in the new form? With some areas I can manage to respond to the prompting section (I have long term mental and physical health problems) but then don't understand what they specifically mean by the separate help question. Does this only mean physical in person help? Has anyone asked the DWP and had a good explanation?

Yes, it does mean help from a person. It's quite possible to say that you need help, but you don't have anyone.
