PIP Review Form was created by Navigator

Hi all,

I have heard that when my PIP award comes to an end the DWP send out a review form. Is this form the same as the lengthy original PIP form, or is it shorter? Also is there a copy of this form available on the B&W website,, or are you considering adding it for future reference.

If not can you say where it maybe located.

The review form is shorter than the original application form. You can read about the review forms from page 133 in the Guide to PiP Claims and Reviews. You can see examples of the review form on the government website.
