Using the answer to PIP AR1 - Activity 1 Preparing Food and Cooking - as an example:
If I use your 6 question format + write “see attached sheets” in the boxes for Questions 25, 26 and 27 and then write my full answer on the attached sheet (as per P138 current Bens + Wk PIP Guide) – do I still need to refer specifically to Q25, Q26 and Q27 as bullet points within the full answer or can I leave it to the DM or Health Professional to glean the info themselves?

You are going to write your answers for Questions 25, 26, and 27 on additional sheets. (That's fine) You are going to write in each box for Questions 25, 26, 27 "see additional sheets (That's fine)
Do you still need to specifically Q25, Q 26 and 27 as bullett points in the full answer. I don't exactly know what you are trying to say here. Whatever you write in your full answer it should be clear which question you are answering and it should be clear that you have answered all parts of it. Do not leave the HP to "glean" information - because they may not do it.
