Advice with questions 19 & 22 on DLA form please. 8 years 2 months ago #66417

I'm looking through the DLA guide and wonder if anyone can expand on the following questions :

Question 19 : Do you have any reports about your illnesses or disabilities?

Can this include occupational therapist reports and consultant / GP letters ( or is it for specific reports only ) ?

Question 22 : Please list the aids and adaptations you use.

Would this include incontinence pads ?


p.s this could be the start of many questions regarding this form :blink:

Another question (I'm keeping this all on the same thread, so no doubt it will be bouncing up and down like a yo-yo !)

In order to put together a comprehensive and fully detailed form that is void of attached sheets (the DWP always seem to lose my attached sheets even when super-glued to the main form).

I'm completing the online PDF DLA app form (typing is my only real way of writing things down) and then printing it out to send via the post.

Any suggestions on holding approx 42 sheets of A4 paper together without using an industrial staple gun :S

by a large roll of sellotape and make full length hinges out it and put on each side of each sheet that way the pages have to stay with the form just like they use to repair school books

Hi Do,

I too used the online PDF ESA50, and printed it off.

My daughter has done all this for me.

She has used the individual clear plastic punched A4 pockets that you buy at any stationers or supermarket.

They only cost less than a £1 for around 25 pockets.

She has then put the 20 page ESA50 and the extra evidence into the individual punched pockets, back to back, and put them all in a file holder, so it is like a book.

She has then done an index to contents at the front of the file, noting each page number, and what is on each page, so pages 1 to 20 would be the ESA50 its self, and then 21 onwards would be any extra evidence, or documents.

In each of the boxes on the ESA50 where information has been continued on one of the A4 pages, she has put "Continued on Document 1, or 2 or 3, etc, at page ? of this file etc etc.

This is then presented as a neat and tidy file, indexed and cross referenced, you can make sure the pages are secure by stapling the top of each of the pockets if you wish.

My file contains 97 pages of ESA50 and extra evidence.

Sorry Do, just noticed that you are talking about the DLA Pdf, rather than the ESA50 that I have quoted, the same principle applies though.
