My doctor or consultant not been asked to fill any form in ?
However i was not happy with operation to be performed, so changed hospitals and consultants.
I have medical in a couple of weeks, but i am waiting to have hip replacemnt, this will not be done for approx 3 months.
I have a letter from new consultant, will this be ok to take to medical with me as evidence.
Thanks

Hi NW,
I would send copy of your new consultant letter to ATOS, at the same address thet you sent your ESA50 to.
I would also send copy to the ESA DM dealing with your claim.
State that you wish it to be used as evidence in your assessment.
It may be advisable to send both these copies by recorded delivery keeping copy.
You can then take copy of the letter to the face to face, as back up.

sorry for the ignorance but who will be the esa dm.
Hi NW,
The ESA DM, is the DWP Decision Maker, who will decide on your award of ESA.
The DM should be based at the Benefits Office that deals with your ESA.
The address should be on any letters that you have regarding your benefit, or on the IB to ESA Migration letter, that you received at the start of the ESA WCA process. ("The benefit you receive is changing", letter)

You can just address your letter to The ESA Decision Maker, at the relevant benefit office/centre.
You could put your name and address on the reverse of the envelope, and put your name, DOB and national insurance number on any documentation enclosed.
Of course keep copy of everything.
