abreviations??? what are they?

can anyone help me with the abreviations on a score sheet for physical activity, and all the other abreviations just to see if i can make any sense of them.

thanks for that I will look through them but they are from a decision makers scoresheet and its somethink like this:-
We, Sc, Rd, Pd, Me, Vd, SPd, Hd, Cc, Fc
and the others are descriptors so not really sure what they mean.

W = Walking

S = Standing and sitting

R = Reaching

P = Picking up and moving

M = Manual dexterity

V = Navigating and maintaining safety (V probably stands for vision)

SP = Making self understood

H = Understanding communication (H probably stands for hearing)

C = Continence

F = Consciousness (haven't a clue what the F stands for)

The small letter refers to the descriptor awarded.
