Submission of 2nd ESA50 whilst Appealing

Hello All,

Today I have finished my second ESA50 form, sent to me 4 months after the decision to put me in WRAG.

I am appealing for SG and am waiting for a date in central London. As advised I put appealing 2 times on the head of two pages in caps and highlighted.

I am waiting on 2 appointments, one which has be cancelled now three times by the Neurology Hospital in London. I do have the cancellation letters.

But, what I don't have (I did inform the GP at my medical the date of my special-specialist appointment (at that time it was 10/5/12) is any NEW information.

I have re-referred myself to PT and OT as the nerve damage which is bi-lateral feet and hands, has caused (?) increased pain and weakness. So at some point should
have a doc from the therapists.

With this in mind, what do I do regarding NO NEW INFORMATION? I must send the ESA50 in soon to make a 7th of December deadline.

1. Do I attach the rescheduled appointment letters?
2. Do I attach the documents they looked at in June??

Any help with this would be appreciated, thank you for reading this.

If you feel that the information to hand will assist with your repeat WCA, then by all means send it with the ESA50.
