G.P. Forms To Be Filled Out For DWP ? ?

Hi,
Please can anyone tell me which form / forms my G.P. has to fill out, to update the DWP about my medical condition.
I understand the DWP send the G.P. these forms, but i would like to know what information they ask for ?

I asked my new G.P. for a letter of support, as well as filling out the DWP form's, but he refused !!
I am very upset and feel very let down, as my previous G.P. did a letter, with no problems at all. :(

Which benefit is this in relation to as different benefits have different forms etc.

Hi,

Your GP may be sent an ESA113.

Re a support letter, it is not within a GP's NHS Contract to supply supporting letters for claimants. Some may do it privately for a fee but others may decline.

They do however contractually have to complete forms sent to them by the DWP.

Emma, have you been involved with any other health care professional recently? I have not seen my GP for at least 5 years, and when the DWP wrote to my GP, the concluded that I did not need any benefit. When I appealed (this was DLA), I included loads of information from other professionals I had seen or support workers etc, and I was awarded higher rate care on the DLA. My welfare rights officer made it clear in my 'reconsideration' appeal that my GP had little understanding of my illness. Now I copy every letter I can to the GP for future reference.

I'm in same boat, Emma. The head of the small practice I go to will only deal with DWP and their policy at the surgery was to not give me a letter of support to send in with my appeal form/letter. I am hoping that the Welfare Rights Officer at CAB may request one, as I think she would comply with that one. I have also started seeing another doctor at the practice who has really been listening to my problems, and I have also given her up-dates about my health, as they hadn't a blinking clue!

Docs are being pushed about, too...but they are there to take care of us and that is what they should be doing.
