When Supplying Additional Information was created by Landlord
Can anybody please advise as to how you presented sheets of additional information when sending back your ESA50. As the boxes they give for providing information are far too small, did you elect to staple the additional sheet of information to the relevant page on the ESA50, or maybe use a paperclip? Or just place the loose sheets within the ESA50 unattached. As I have read that the forms are opened in a Royal Mail sorting office, and then passed on to the relevant address, if this is true then inserting loose sheets of additional information could be a little risky.

Not wishing to upset the decision maker I wish to present the ESA50 form in the best possible light. I will have about 20 extra sheets to send with the form, which will include additional information and doctors and specialist letters.

Any feedback on how you presented your ESA50 form would be much appreciated.

Hi LL,

As your query relates to a generic question regarding the ESA50 Questionnaire, and is not pertaining to ESA claimants in France, I will try to advise.

The most important issue is to ensure that any seperate sheets are secured as well as they can be to the main ESA50.

Maybe you could use see through hole punched plastic pockets, and put the ESA50 and the seperate sheets into said pockets, and then put them into some kind of file holder.

It would help if you numbered any seperate sheets, e.g. 1 of 4, 2 of 4, 3 of 4, 4 of 4, etc, etc, and drew reference to each seperate sheet in the applicable section of the main ESA50, e.g. see 2 of 4, etc etc.

Ensure that you put you name, DOB, and National Insurance Number at the head of any seperate sheet, at least then if it becomes detached, it will be apparent where it belongs.

You could compile an Index, and also put in a covering letter stating how many seperate sheets are enclosed to accompany the ESA50.

These are just some pointers that may help, I am sure that any members who have done the same will post comments if they can.

Hi Landlord

I very carefully holepunched the ESA50 so that the two holes didn't go through any words, it took me a few minutes to align the thing properly before I punched but the caution paid off. Then I holepunched all my additional sheets and used treasury tags in the holes to keep everything together. As for the boxes, I referred to each paginated sheet and directed the reader to the additional sheets that way - I didn't start any of my written answers in the boxes other than this redirection because I assumed it'd be irritating to the reader to start reading the answer on the form, then have to shuffle through and find the right page at the back to continue reading my answer. At the front of the form (page 3?) I explained how all the extra sheets were arranged in sections (about you;meds;hospital/physical/mental/supporting medical evidence) with the number of pages for each section. Basically, I made the whole thing as reader-friendly as poss, knowing that they have to get through a fair few every day.

Having said all that, it may not make a blind bit of difference to how they assess my claim, but I had to know I did my best for myself (with a considerable amount of help and support from someone else).

Hi LL,

It was far easier in my own case, as I used the Pdf ESA50 as here : Use Of The Interactive PDF Version Of The ESA50 Questionnaire

I then just printed off the ESA50 as A4 pages, added my seperate pages of evidence as in my previous post, and compiled a very neat, indexed, cross referenced file with a covering letter.

Of course, I live in the U.K.

Hi Landlord,

Just saw this post. I stapled a few pages to the last page. I put my name , address and national insurance no. on each page.

None of this information arrived with my ESA50 form in France. I took all the copies to the assessment and the french doctor read them. Not sure why they were parted from my original form. Some were in French as they were results. Maybe they had been unstapled to be translated and not put back together.

Keep copies of everything in case you need them again.
