Recording WCA. Broken Link

Hello,

I was just about to print out the Template letter Requesting that a Face-to-Face assessment might be recorded, from.....

Unfortunately, the Link quoted within this letter appears to be incorrect :

I need to get my ESA50 sent off very soon. Therefore, would you advise that I simply remove this Link and the reference to it from the letter, or do you have the correct Link?
