dla form was created by cw12
hello could anyone advise me concerning dla form. i have filled in most of the form and read the guides. my query is, is it better to fill in the form online or handwrite it, would either go against your chance in some way. also i get help from all my family but am unsure weather to put this on the form as they do not want to have to be questioned by dwp, is it best to leave that section out of the form or could i put that my family help me and leave there personal info out, also same as to regards to help with filling in the form. any advice would be great, with thanks.

Whilst this is not ESA, where one of the descriptors specifically relates to this area, you should none the less give it some thought, and if necessary, include a few words to explain your choice.
You do not need to supply any contact information for your family, but if you need their assistance I would certainly encourage them to allow you to reference their help on the form.
