I assume you are talking about PIP replacing DLA.
Everyone will be re-assessed, in some cases this may also involve a medical, but it is very unlikely that this will be applied in all cases.
Using the IB/IS/SDA transfer to ESA as an example, a lot of people were transferred based on their prevous medical evidence without their being required to attend a WCA assessment.
