If I was to send a SAR, what address would I need to send it to?

You should be able to make the request to the office that normally deals with your claim.
There is a guide to SARs in the Members Area.

I am so pleased for my outcome and wold like to say a big thank you to B&W.
I have been thinking about Universal Credit which appears to be starting in April 2013.
Will I have to go through the process to qualify for UC?

As you are receiving ESA(CB), having been transferred from IB, you should not be affected by UC as it is only ESA(IR) claimants that will be transferred.

I received my ESA85 Today.
Majority of it was OK but a few dubious remarks.
It states "The client was found to meet the support group criteria"
at the end it states "The available evidence suggests a review in the medium term"
Exactly what would "Medium Term" mean?
