List of support group discripters and Atos email a

I s there a list of the new support group discripters for the NEW 1/13 ESA50 that I could print off and take to the CAB. Sorry for asking this but my head is very foggy I have had a nasty virus for the last 3 weeks just when I want to be clear headed to fill in the form. I think I'm going to make a mess of it.

Also could you tell me the Atos email address which I've also lost in case I can't get the form back on time. It has got to be back on 26th march and I'm thinkingI'll have to post it special delivery next friday 22 march as it won't be finished before then.

Please help as I'm getting a bit desperate IB to ESA conversion.

I would encourage you to do your utmost to get the form submitted on time. There is no legislation that allows for the time to be extended, no matter what you might be told if you ring the DWP helpline!
